Same-level slips and trips are the falls that quietly fill the injury log, and they yield to a layered fix that starts with the floor and ends with the footwear. In this article, we’ll look at where these falls happen, a Michigan case that shows both halves of the control, how to work the layers in order, why the wet-floor sign sits near the bottom, and where to learn more, all anchored to OSHA’s walking-working surfaces rule, 29 CFR 1910.22.
Where Same-Level Falls Actually Happen
Falls from racks and ladders get the attention, but they are not what fills the injury log. The everyday fall is flat: someone crossing a floor hits a surface that lost its grip while carrying something that leaves no hand free to catch themselves. It lands the same way whether the floor is a pick aisle, a wet processing line, or a hospital corridor. These cluster where contamination meets traffic and people move with their eyes up — the dock apron and freezer threshold in a warehouse, the wash-down zone on a food line, the sink and ice-machine area on a unit. A same-level fall is rarely one thing; it is usually a floor that lost traction meeting a boot that lost tread, at the worst possible moment.
A Fall That Shows Both Halves
A Michigan case makes it concrete. In its investigation of Case 523, the state’s Michigan Fatality Assessment and Control Evaluation (MIFACE) program described a commissary worker carrying an empty tray across a concrete floor kept damp by the refrigerated space. He slipped, fell backward, hit the back of his head, and died a week later. The floor was textured to resist slipping and the company required slip-resistant shoes, but investigators found the soles of his shoes had worn smooth over time. The floor and the footwear are two halves of one control: a slip-resistant sole worn smooth is no longer slip resistant, and a worker with hands full has nothing to break the fall.
Fix the Floor Before the Worker
Where the wet or the clutter is the normal state of the room, the control has to be built into the floor, not into a moment of attention that fails under production pressure. OSHA’s walking-working surfaces rule puts the same idea plainly; 1910.22(a)(2) requires:
The floor of each workroom is maintained in a clean and, to the extent feasible, in a dry condition. When wet processes are used, drainage must be maintained and, to the extent feasible, dry standing places, such as false floors, platforms, and mats must be provided.
Work the layers in this order.
- Drain it and pick the right surface: Slope the floor to trench or point drains so water and product leave the walking area, and keep the drains clear. Then specify a surface that grips when wet. The recognized target for a floor walked on when wet is a wet dynamic coefficient of friction (DCOF) of about 0.42, measured under ANSI A326.3. But 0.42 is set for water: a rendering line, a wet-cook area, or a fryer zone is wet with fat and oil and needs a more slip-resistant surface, so match the floor to what actually lands on it. In a clinical setting, seamless welded vinyl with coved edges keeps dry more easily than tile with open grout lines.
- Lift people onto dry standing places: False floors, raised platforms, and drainage mats or duckboards at fixed wet stations get the worker up off the film. Maintained, they are a real control; left to curl, clog, or slide, they become a trip hazard, so they go on the same walk as everything else.
- Keep the housekeeping honest: Contain splash at the source, squeegee and pick up product fast, and keep wet cleaning away from traffic by timing and signage. On a clinical floor, mop one side of a corridor at a time so there is always a dry path. This is the layer most likely to lapse when the line is behind, which is exactly when it matters.
- Slip-resistant footwear, last and never alone: Good outsoles cut slips, and the evidence is strong — a National Institute for Occupational Safety and Health (NIOSH) trial of about 17,000 food-service workers across 226 school districts linked free, highly rated slip-resistant shoes to a 67 percent drop in slip claims. Worth funding. But that still leaves about a third of the slips, the tread wears out, and a shoe only ever helps one pair of feet. It backs up the floor; it does not replace it.
The Wet-Floor Sign Is a Backup, Not the Fix
Notice the sign sits below cleaning the floor, not above it. A wet-floor sign warns about a hazard that fast cleanup would have removed. The strongest habit is that whoever spots a spill owns it until it is dry or guarded, rather than leaving it for whoever gets to it, which is the exact gap these falls drop through. Use signs and mats to manage the water you cannot get to yet. Make the reporting path frictionless too: a worker who spots a recurring wet patch by an ice machine or a leaking fixture should be able to raise a work order as easily as housekeeping mops a spill. A transient spill is a stop-and-fix; a worn floor finish or a bad drain is a scheduled repair. Both have to happen before the next person crosses, because the next person is not looking down.
The Owned Shift Walk
Turn all of it into a walk with a name on it. Once a shift, someone crosses the floor with the authority to pull an area out of use until it is safe. Run this check every time:
- Spills: Wiped up now, with signs and a kit staged out on the floor, not in a back closet.
- Walkways: Clear of strapping, stretch-wrap tails, broken pallet boards, cords, and anything that slid off a rack or cart.
- Drains and mats: Drains flowing; mats flat, clean, and not curling; dock aprons, wash-down areas, and freezer thresholds kept dry and marked.
- Finish and soles: Floor finish not worn smooth, and boot soles checked for tread, because a slip-resistant sole worn smooth is just a smooth sole.
- Anything left over: Whatever you cannot fix on the spot roped off before the next person walks it.
Then re-check the floor itself: retest the grip when you resurface, when a new process changes what the floor gets wet with, or after a run of slips, so the number under people’s feet is current. A floor specified for the mess it actually sees, drained so the mess leaves, and walked every shift is what stops the same-level fall from being the most predictable injury in the building.
